Applicants need to submit required document copies (including but not limited to the following documents): Valid address proof, including: (a) Utility bills issued within the last three months; (b) Correspondence issued recently by a government department or agency (i.e., issued within the last three months); (c) Statements or notices issued by a bank, licensed corporation or insurance company in the last three months. Valid income proof documents, including: (a) The most recent tax bill; (b) Pay slips or employer's mandatory provident fund contribution records in the last three months; (c) Bank account passbooks/monthly statements showing salary deposits in the last three months; Copy of your and the supplementary card applicant's (if applicable) Hong Kong Resident Identity Card/Hong Kong Permanent Resident Identity Card/Travel Document to and from Hong Kong and Macau, and original place of residence Identity Card; If you are a full-time university/college student and are studying for a certificate or higher program issued by the following institutions or their affiliated colleges, including: recognized universities in Hong Kong, Zhuhai College, Vocational Training Council, Hong Kong Institute of Professional Education, Hong Kong Academy for Performing Arts, Caritas Institute of Higher Education, Caritas Bianchi College of Careers, Hong Kong Institute of Technology, Hong Kong College of Technology/College of Technology, Ming Tak College, Tung Wah College, University of Wollongong in Hong Kong, Caritas Community College, Hong Kong Art School, Gratia Christian College and Yew Chung College of Early Childhood Education, please attach a proof document of Hong Kong Resident Identity Card / Hong Kong Permanent Resident Identity Card / Passport (if applicable) and a full-time university / college student ID.
